Licensed and Rapidly Growing Home Care Agency serving the 4 Boroughs of NYC and Long Island is seeking an EXPERIENCED Home Care Intake Coordinator. Location for this role can be our Elmhurst, Queens location or Corporate Office in Merrick. Candidate must understand the regulations governing the Home Care field, related to Medicare, Medicaid and other insurance and is committed to excellence in patient care. The Intake Coordinator will work independently and will also be a part of a supportive Intake Director.
POSITION SUMMARY
The Intake Coordinator is responsible for processing referrals received from marketing sources such as community outreach marketers, hospitals, nursing homes and other affiliated partners. The Intake Home Care Coordinator will contact each patient to educate them on the services the agency can provide and assist them with their home care needs for services. The Intake Coordinator will accept the responsibility of following a patient from the inception of the inquiry, through enrollment into long term care (MLTCP/MCO) or CHHA services. The Intake Coordinator shall follow up with the patient/family to keep them always informed as to the status of their enrollment.
REQUIREMENTS
- At least TWO Years experience in home care intake with a licensed home care service agency
- Understanding of communicating effectively with employees, patients and their families, medical and community affiliates in order to develop positive relationships.
- Proficient in HHA Exchange, Microsoft Office Suite and Microsoft Excel
- Investigative ability, highly organized, self-motivated, takes initiatives, excellent written and verbal communication skills; able to perform with minimum supervision.
- Organized and Punctual
- Bilingual Spanish a plus but no required.
JOB RESPONSIBILITIES
- Receive referrals from referral sources
- Verify patient's insurance to determine eligibility
- Educate patients on provided services
- Send referral to MLTC for potential enrollment or Medicaid Assistance
- Send referral to appropriate CHHA for skilled services
- Determine which services were approved by insurance
- Ensure PHC receives authorization for MLTC cases and provide case specifics to PHC HHA Coordination Department.
