We are seeking an experienced and detail-oriented MDS Assessor to join our team. The MDS Assessor is responsible for conducting and coordinating resident assessments and completing MDS forms in compliance with State and Federal regulations and facility policies. This role is vital to ensuring accurate documentation, quality resident care planning, and regulatory compliance.
Compensation:$130,000-$145,000
Key Responsibilities:
Conduct and coordinate thorough and accurate resident assessments for Medicaid and Medicare.
Review daily census to identify required assessments.
Prepare interim schedules and significant change assessments as needed.
Input data for Medicaid and Medicare MDS forms into American Health Care Systems.
Prepare and transmit MDS/PRI files to the Department of Health using IBM Dialer and Netscape, ensuring timely and accurate submission.
Generate and review Quality Indicator reports from state software as required.
Maintain organized records by filing MDS booklets in resident charts.
Order and manage supplies for the MDS office.
Input and update PRI full house data, quarterly assessments, and discharges.
Distribute reports to leadership and collaborate with the Quality Improvement Department.
Ensure compliance with regulatory guidelines and facility standards.
Perform other related duties as assigned.
Qualifications:
Bachelor of Science in Nursing (BSN)
Minimum of five (5) years of clinical nursing experience, including at least two (2) years performing MDS completion
Current and active New York State Registered Nurse (RN) license
