Your contributions will be vital in helping our residents achieve their rehabilitation goals, always adhering to established legal, state, and federal standards and best practices. **Essential Functions:** + **Implement Personalized Treatment Plans:** Execute physical therapy modalities and interventions for individual residents as outlined in their personalized treatment plans, meticulously designed by the Physical Therapist, ensuring all care is in accordance with physical therapy standards. + **Monitor and Document Patient Progress:** Observe, accurately record, and effectively report each patient's condition, progress, and reaction to treatment to the supervising Physical Therapist.
