Resident Assessment Nurse (RN) – Webb City Health and Rehab

About the Role

Under the direction of the Director of Nursing (DON), the Resident Assessment Coordinator leads an interdisciplinary team using a core set of screening, clinical, and functional status elements to form the foundation of a comprehensive resident assessment. These assessments guide the Care Plan, ensuring it reflects the resident’s wants, needs, and goals.

Key Responsibilities

1. Coordinate the interdisciplinary assessment process for assigned residents
2. Oversee development and completion of Comprehensive Care Plans in line with company, state, and federal guidelines
3. Monitor residents to ensure Care Plans are followed and updated as needed
4. Report any variances to the DON and Care Plan Team

Qualifications

1. Current RN license in good standing (State-required)
2. MDS experience preferred, but not required – we will train!
3. Knowledge of general nursing standards and medical practices
4. Strong critical thinking, assessment, and documentation skills
5. Computer literacy and ability to use electronic documentation programs
